The TOEVA Membership Program is a family protection plan designed to help protect against unforeseen expenses related to ambulance transport. For just $40 a family can have the peace of mind knowing that they will not be billed any additional cost if their insurance does not cover the full expense of ambulance transport to the hospital.
This holds true even if your family member needs more than one transport through the course of the year.
Here are some frequently asked question regarding our Plan.
Who is eligible for membership? Membership is open to residents of Three Oaks and Galien Township and Village. Family members who can legally be claimed on your federal income tax forms are considered eligible family members.
If I have insurance, who receives payments? The TOEVA Membership Program is not insurance, TOEVA will pursue claim payments from your insurer or Medicare. We submit the claim for you. If your insurance company makes payment direct to you, it is your responsibility to make payment to us.
What if I don't have insurance? If you do not have insurance you will be billed a predetermined amount for services rendered. Typically this runs about $200 and it is your responsibility to pay it.
